Please complete the following 3 steps to ensure your transfer from Dr. Linda Wong to Dr. Farahnaz Daya is completed successfully:

Signed Roster Form - Before September 9, 2022
To officially be enrolled with Dr. Farahnaz Daya, please sign a Ministry of Health Roster Form and return it to Citrus Medical Centre as soon as possible. The roster form must be received prior to September 9th to guarantee a spot with Dr. Daya. Any forms received after the due date will be processed based on availability.

Please note, you will not be able to book with Dr. Daya until after September 19, 2022. All patients who have returned a signed roster form will be emailed further instructions closer to September 19, 2022. For any appointments needed prior to this date, you can still schedule an appointment at Prince Edward Medical Centre.

Access Dr. Daya's roster form from one of two options below:

Please complete a separate form for each patient age 16+. Children age 15 or younger can be added to the same form as a parent or guardian.
Click the link and enter your email address to be sent a digital electronic form from Hellosign, our digital form signing platform!

Once you have filled in all the required sections, click Continue in the top right corner. Then click "I Agree" to submit the form directly to Citrus Medical.

Download the PDF to attach a digital signature or print & sign physically. Submit the signed form to Citrus Medical:

If you would like to have your complete medical record transferred to Citrus, please contact Records Management Ltd at 1-800-775-0093 or fill out the form below:

Please note, it is NOT mandatory to have your medical records transferred to Citrus Medical to become a patient. You can still be enrolled with Dr. Daya without your previous records.

Which location is Dr. Daya located at?

Dr. Daya will be working out of the 605 Royal York Road (Queensway and Royal York).

How do I schedule an appointment with Dr. Daya?

Before September 19, you will not be able to schedule an appointment with Dr. Daya.

After September 19, the fastest way to book an appointment is using our online booking portal, Health Myself! You will be able to access Dr. Daya's schedule to book in-person or virtual visits, and book into our Urgent After Hours clinic. Read more below on how to set up online booking.

Setting up Online Booking - After September 19, 2022
Our online booking portal, Health Myself, will allow you to schedule appointments directly with Dr. Daya, or our Urgent After Hours clinic!

Click the button below to login for the first time as a "guest". Once in, click Dashboard in the top right corner and Finish Registration to set up your profile.

By completing your registration, you will now be able to login anytime using just your email and password to book, change, or cancel appointments!
