Fee for Form Completion
Most forms are NOT covered by OHIP and a fee will be charged for its completion by a Citrus doctor. The exact fee will be provided upon completion as the fee is dependent on the complexity and requirements of the form. See below for approximated rates for common form requests.
When requesting to have a form completed by a Citrus doctor, complete the following to ensure your form can be returned to you in a timely manner:

Read the entire form and prepare answers to all the questions.

Fill out any information that is required by you (i.e. personal information & details, your signature, etc.)

Submit the document through our Contact Form at least 48 hours prior to your appointment OR bring the form with you to your in-person appointment
Please allow 3-7 business days for the form to be completed. Additional time may be required if lab results are needed. If there is an urgency to the form, let us know and we will try our best to accommodate.
