Many medical services fall outside of OHIP, including sick notes, insurance forms, work absence forms, certain injections, and prescription renewals without a visit. These are known as Uninsured Services and are the patient’s responsibility.

The exact fee for forms is dependent on the complexity and requirements of the form. The final fee is at physician’s discretion upon review and completion of the form. See the PDF linked below for approximated rates.

By Phone: Credit card payments can be made over the phone. Please call us at 416-800-6500.

Interac e-Transfer: Send your payment to info@citrusmedical.ca. Be sure to include the patient’s name, date of birth, and purpose of payment in the message.
