Message from Dr. Sylvia Lee:


Dear Patients,

I am writing to let you know that I will be leaving Citrus Medical Centre as of Feb 28, 2026 for personal reasons. It has been a true privilege and honor to serve as your family physician, and I deeply value the trust you have placed in me over the last three years. Read below for next steps and how to transfer your care to another physician at Citrus Medical. Multiple physicians, both male and female and at both locations (605 Royal York Road and 2240 Lakeshore Blvd), have agreed to take on my patients. You will have the option of selecting which physician to continue your care with.

What You Need to Do

Please complete the secure form we sent you as soon as possible so we know how you would like to proceed. You would have been emailed with a Secure Ocean e-form emailed from no-reply@citrusmedical.ca with the subject line: “Action Required: Dr. Sylvia Lee Departure from Citrus Medical

For patients without access to email or this form, please call 416-800-6500 ext 5 so our staff can record your choice.

Deadline: If we do not receive your completed form or response by Feb 21, 2026, we may not be able to guarantee a transition to another Citrus physician.

Your Options (Choose One on the Form)

Medical Records

If you are transferring your care outside of Citrus Medical:

  • Standard medical record transfer fees apply (based on number of pages in your chart)

  • A signed consent form and payment is required before records can be released.

If you already have a new physician:

  • Please submit a signed Request for Health Records form (from our website or your new clinic).

  • We will contact you with the final fee once your request is processed.

Finding a New Family Doctor

If you do not wish to stay at Citrus and need a doctor elsewhere, you can contact Health Care Connect at 1-800-445-1822 or at ontario.ca/healthcareconnect

Booking a Transfer Intake Appointment

To gain access to available intake appointments, you must either complete the Ocean e-form or notify the clinic that you intend to remain at Citrus Medical with another physician. Booking access is only enabled after one of these steps is completed. If you have already done so and have not received further instructions within 3–5 business days, please contact the clinic for assistance.​

Please Note The Following Important Information:​

  1. An in-person intake appointment is required to secure your spot with a new Citrus Medical physician.

  2. All patients aged 16 and older must have their own separate intake appointment. Children under 16 do not need a separate intake appointment if their parent or guardian is a Citrus Medical patient. Please let your new family doctor know which child or children are part of your family during your intake appointment.

  3. If your intake appointment is AFTER Feb 28, 2026, you will continue to have access to the Citrus Medical Urgent After-Hours Clinic and Dr. Lee’s limited schedule for emergencies until your intake appointment.

  4. Please DO NOT call to book an intake appointment. Our team has access to the same schedule that is available through the online booking portal. Online booking syncs in real time, including any cancellations. We are happy to assist with technical difficulties. However, if you do not see a physician listed as an option to book, that physician is not accepting new patients. If there is a physician that no longer has available appointments, that physician is no longer available.

  5. Appointments are first-come, first-served, and not all Citrus Medical physicians are accepting new patients. Additional intake appointments may become available in the coming months, but this cannot be guaranteed. We strongly encourage you to book as soon as possible to secure a spot.

Important Reassurance About Your Care

All patients who have indicated that they wish to remain at Citrus Medical and who have completed the form before the deadline Feb 21, 2026 will be guaranteed attachment to a family physician at Citrus. Physician selection and intake appointment dates are offered on a first-come, first-served basis. However, if intake appointment slots are no longer available please do not worry. We will be matching you and all Citrus patients with a Citrus Medical physician.​

We greatly appreciate your cooperation during this transition. We understand that this can be a stressful time, and our goal is to make this process as smooth and fair as possible for all patients. We will continue to keep our website transition page updated with any pertinent changes or new information. Please be patient and respectful with our staff as they work to assist everyone as quickly and efficiently as possible.

Verbal abuse, harassment, or threatening behaviour toward our staff will not be tolerated and will result in dismissal from the clinic.
